Advance Directives

An advance directive is a document which tells others what your choices are about medical treatment, or it names someone to make decisions about your medical treatment in case you lose the ability to make decisions yourself. We are required by law to ask if an advance directive has been made. If you have a living will or another advance directive, we need to have a copy of the document in our files to be sure your decisions are known and honored.
